図書館で調査・回答できる範囲での各種質問および図書館への要望等を受け付けています。
回答が必要な方は「氏名」「E-mailアドレス」「連絡先」を必ずご記入ください。
E-mailでの回答を原則としています。メールアドレスをお持ちの方は必ずご記入ください。

We accept various questions or requests to the library as far as we can investigate and reply them.
If you need an answer, please make sure to fill in "Name" "E-mail" "Address".
We generally reply by E-mail. When you have E-mail address, please fill in "E-mail".

所　属 / Affiliation	選択必須 Required item
E-mailアドレス / E-mail	(半角90文字まで) (90bytes)
連絡先(電話番号等)/Address(e.g., phone number)	(半角120文字まで) (120bytes)

タイトル / Subject	入力必須 (全角256文字まで) Required item (512bytes)
質問・要望等の内容 / Comments	入力必須(全角1024文字まで) Required item(2048bytes)
	調査済の資料等がありましたらお書きください。 If you have researched on your own, please indicate the result and the materials you have used.
